New Student Packet

Please fill out the following forms as soon as you are able. In order to register for fall semester courses, incoming students must complete and return these forms no later than June 30, 2025.

  • Please note that the health form must be printed and filled out manually. Part of the health form must be filled out by a physician. Once completed, the health form should be scanned and emailed to donna.farmer@drbu.edu.
